The Medical Record Room & Statistics Department is responsible for the systematic custody, retrieval and authorized issuance of hospital medical records, and the compilation and analysis of hospital statistics to support patient care, institutional planning, research and decision-making.

Mission
To maintain medical records with due care and confidentiality, and provide accurate, timely statistical information in coordination with relevant departments and HMIS.
Key Functions
- Maintain, index, preserve and retrieve patient files under departmental custody.
- Facilitate authorized file issuance and requests for certified copies in accordance with applicable procedures.
- Maintain appropriate records of file movement and requests.
- Compile, validate, analyze and report hospital statistics received from concerned departments/HMIS.
- Support preparation of periodic statistical reports and the PIMS Year Book.
- Coordinate with HMIS and relevant departments to improve data quality, reporting and record-management processes.
- Implement approved record-retention and disposal procedures under directions of the competent authority.
Services
Authorized medical-record retrieval/issuance; processing of eligible certified-copy requests subject to verification and approval; and compilation of hospital statistical information for authorized official use.
Confidentiality
Patient records and related information are confidential. Access, handling and disclosure are restricted to authorized purposes and must comply with applicable rules, institutional policy and approvals.
Modernization Priorities
Improved file tracking and indexing, standardized statistical reporting, coordination with HMIS, and digitization initiatives where approved and resourced.
